By participating in activities at the Playtime Ronoke Inc. Indoor Playground (referred to as "the Company"), the undersigned, on their behalf and on behalf of their executors, heirs, successors, and assigns, acknowledges and agrees to the following:

1. Assumption of Risks: The undersigned knowingly and voluntarily assumes all risks, both known and unknown, associated with participation in activities at the Playtime Ronoke Inc. Indoor Playground. These risks include, but are not limited to, injury, property damage, or other incidents resulting from participation.

2. Release of Liability: The undersigned releases and holds harmless Playtime Ronoke Inc., including its officers, employees, agents, representatives, and affiliates, from any and all claims, damages, or liabilities arising from death, injury, disability, property damage, theft, or other incidents, whether caused by negligence, equipment issues, or the actions of other participants.

3. Physical Fitness Certification: The undersigned certifies that they and their children are physically fit to participate in activities at the facility and have not been advised otherwise by a qualified medical professional.

4. Compliance with Rules: The undersigned agrees to follow all posted rules and conditions provided by Playtime Ronoke Inc. staff. In case of any hazards or injuries during participation, the undersigned will promptly notify an employee.

5. Acknowledgment of Waiver Use: The undersigned acknowledges that this waiver and release form will be used and relied upon by Playtime Ronoke Inc. to govern participation and related rights.

6. Signature Requirement: Signing in and providing a signature is mandatory before utilizing the facility.

7. Unpaid Balances: Any unpaid balances may result in legal action for recovery.
